Overview

ACORN Health Assessment (1st & 2nd Appointments) This bundle is for 2 appointments: 1) The Intake Appointment you are scheduling now (3-hours long) 2) The Follow-up Appointment you will schedule with your clinician (1-hour) Your Intake Appointment is the first appointment and the longest. This is where your clinician will listen to your whole health history. They will ask you detailed questions and find out as much information as they can to better understand your current state of health. This appointment takes between 2 and 3 hours. ​​​At the end of this appointment, you will schedule the Follow-up Appointment with your clinician. The Follow-up Appointment is the second appointment in this bundle and is 1-hour in length. Before this appointment, your clinician will spend time researching your health concerns and creating a custom health plan for you. These findings will be discussed with you in the Follow-up Appointment. Once you have completed the Intake & Follow-up Appointments, you are now an established client of the ACORN Clinic and are eligible for ACORN Care Appointments.
