What is Collaborative Care Clinic?

Our Collaborative Care Clinic is a team of certified Herbalist Clinicians who have graduated from the ACORN School's Clinic Program, having completed over 1200 hours of formal training and education in herbs, nutrition, and Vitalist therapeutics. They provide telehealth clinical services through the ACORN Clinic. This clinic platform allows clients the option to select from a variety of practitioners and appointment times, granting access to receiving faster care for our services.

​​Our senior faculty, Heather Luna, will review, monitor and advise consultations. Heather is the Founder and Director of the ACORN School of Herbal Medicine, and she is a Clinical Herbalist and Clinical Nutritionist with over 25 years of experience in herbal, nutritional, and vitalist practice and teaching.

About your consultation

Your consultation is part of our “Professional Network of Care.” Each client case that comes to our clinic is peer-reviewed by the Clinic Team for collaborative feedback, accountability, and safety of our methods. While you will work directly with the practitioner of your choice, your clinician will report your case to the clinic team for collaborative case review. We work together to unravel potential root causes and devise appropriate treatment plans. Collaborative care health services improve patient outcomes, enhance quality of services, providing better treatment of chronic conditions, and lead to greater client satisfaction. This collaborative and comprehensive process of review and evaluation, provides you with the best health plan possible. Your health history and medical information will be discussed anonymously to preserve client confidentiality.

How our clinicians help?

Our ACORN Clinicians are thoughtful, thorough, and have a true desire to help people feel better. They take time and care in preparing custom health plans for their clients.

 

These health plans do not rely on herbs alone. Some of the most important aspects of health are related to how well we care for ourselves. Are we getting enough sleep? Do we take time to move our bodies? Are we getting good nutrition to help fuel our days? Our Clinicians take the time to listen to your health journey and find ways to support healthy changes. Skillful applications of herbs can help nudge the body back to a state of balance.  

  • Our clinic is for non-emergency health concerns.

  • We work with herbs, nutrition and lifestyle changes to educate and empower individuals to take charge of their own health

  • All herbal formulas and remedies we recommend are offered at a discounted price to you.

  • All appointments are virtual.

Getting Started
Initial Intake + Follow-up Bundle

To get started as a client of the Collaborative Care Clinic, you must complete the Initial Intake & Follow-up appointments first. 

1st Appointment (Initial Intake)

Duration: 2-3 hours long

Your first session with us is designed to be a thorough and detailed intake of your health concerns and comprehensive interview to obtain your health history. This process allows us to get to the root of your health concerns and account for the individual uniqueness of your case.


 

2nd Appointment (Follow-up)

Duration: About 1 hour long

After the Initial Intake appointment, your clinician will research your case and perform an extensive analysis to reveal patterns and root causes that may be related to your health concerns. This information will then be reviewed by peers and faculty for feedback. Your clinician will present these findings to you during this 2nd appointment (Follow-up). You and your clinician will then be able to discuss detailed treatment options to address your health concerns.

Costs

Initial Intake + Follow-up Bundle

$720

 

When you book the Initial Intake appointment you will be purchasing both the Intake and the Follow-up appointments. You will schedule the date of your 2nd appointment (Follow-up) with your clinician at the end of the Initial Intake.

Ongoing Follow-up Appointments

$180 per hour


We see the most profound experiences from continued care over the course of several months. Once you complete the Intake Bundle appointments, you are an established client and can schedule regular appointments with your clinician whenever you would like. Contact your clinician to book an appointment.

Payments & Policies

Payments
Due in full at the time of booking. We only accept credit cards as payment. 

 

Refunds
We do not offer refunds. 

 

Cancellation Policy
Contact your practitioner as soon as you know you cannot make an appointment. 

 

Here are your options if you need to cancel:

  • More than 48 hours notice: You can reschedule your appointment. 

  • Less than 48 hours notice: Your appointment will be cancelled without refund. You may purchase a new appointment.
