CLINIC Intake Bundle (Saylor)

Overview

Initial Intake & Follow-up Bundle The cost of this bundle is for 2 appointments: 1) The Initial Intake Appointment you are scheduling now, and 2) The Follow-up Appointment you will schedule with your clinician at a later date. Your Intake Appointment is the first appointment and the longest. This is where your clinician will listen to your whole health history. They will ask you detailed questions and find out as much information as they can to better understand your current state of health. This appointment takes a maximum of 3 hours. ​​​At the end of this appointment, you will schedule the Follow-up Appointment. The Follow-up Appointment is the second appointment in this bundle and is 1-hour in length. Before this appointment, your clinician will spend time researching your health concerns and creating a custom health plan for you. These findings will be discussed with you in the Follow-up Appointment. Once you have completed the Intake & Follow-up Appointments, you are now an established client of the ACORN Clinic and are eligible for ACORN Ongoing Follow-up Care appointments.  * * * Tip - Become a Site Member of ACORN by creating an account.
